Healthy Rewards WELLBEING INCENTIVE PROGRAM

Nestlé’s wellbeing incentive program offers opportunities to earn money for completing healthy tasks to reduce your medical paycheck contributions.

FREE MONEY WITH HEALTHY REWARDS

Nestlé now partners with CareFirst WellBeing, bringing you a fresh, dynamic wellbeing experience. And the best part? You can reduce your 2027 medical paycheck contributions by earning Healthy Rewards — up to $400* if you cover yourself, or up to $800* if you also cover a spouse or domestic partner!

ELIGIBILITY

All Regular, U.S. NesCARE benefits-eligible employees (Hourly regularly scheduled to work 30+ hours or Salaried working 20+ hours, as defined in the NesCARE SPD) and spouses/domestic partners on file.

Required: Register with CAREFIRST WELLBEING

You are REQUIRED to register for a CareFirst WellBeing account to be eligible to receive any Healthy Rewards credit.

EARN REWARDS

Choose any combination of these options in your CareFirst WellBeing account for a total of up to $400. If you cover a spouse or domestic partner, they can create their own CareFirst WellBeing account and complete Healthy Rewards activities for an additional $400.* (You can complete all activities to support your wellbeing, but an individual can only earn up to $400.)

Choose from one of the options below to get your biometric screening before November 30, 2026. If you have already had a screening with your health care provider/primary care physician as of December 1, 2025, you can utilize the Primary Care Provider Form found within your CareFirst WellBeing account, then manually enter your results within the Biometric section under Healthy Rewards.

Follow the steps below to choose your biometric screening option. Log in or create your CareFirst WellBeing account and follow the steps below to get started.

  • Click on the Rewards button or the Healthy Rewards tile on the home screen.
  • Choose Complete a Biometric Screening.
  • Click View Screening Options.

You can choose one of the following options to get a health screening:

  • Your health care provider’s office — Talk to your provider beforehand about fasting guidelines. Take a copy of the Primary Care Provider Form from CareFirst WellBeing to your appointment. Screening results from doctor visits/health care providers will only be accepted from December 1, 2025, to November 30, 2026, due to program deadline dates. Once complete, you must manually enter your results in your carefirst.com/wellbeing account. Go to Achieve > Rewards > Complete a Health Screening section. Click Enter Your Results next to View Screening Options.
  • Onsite screening (employees only) — Not all locations are eligible for onsite screenings. If you don’t see your location on the website, contact your local HR department for additional information or choose another screening option. Your results will automatically be uploaded to CareFirst WellBeing, but you must register for a CareFirst WellBeing account by November 30, 2026, to earn Healthy Rewards.
  • Quest Diagnostics Patient Service Center — Pre-register through your CareFirst WellBeing account for a screening at a Quest Diagnostics Patient Service Center near you. Remember to fast and bring a photo ID to your appointment. Results will automatically be uploaded to CareFirst WellBeing.
  • CVS MinuteClinic — (CareFirst Members Only, Fasting Recommended) Access the CVS health screening form from your CareFirst WellBeing account to print and take with you. Bring your CVS MinuteClinic form, photo ID and CareFirst member ID card. Appointments recommended.
    • For screenings performed at a CVS MinuteClinic location in MD, D.C. or Northern VA, your results will be automatically uploaded.
    • For screenings performed at a CVS MinuteClinic location outside of MD, D.C. or Northern VA, you must manually enter your results in your CareFirst WellBeing account under Rewards in the Complete a Biometric section (Click Enter Your Results next to View Screening Options). Screening results from CVS MinuteClinic will be accepted from March 1, 2026, to November 30, 2026.

Complete the RealAge Health Assessment online between January 1, 2026, and November 30, 2026, to understand more about your health and get personalized suggestions to help with your overall wellbeing.

The RealAge Health Assessment is confidential. Nestlé does not see individual results.

Log in to CareFirst WellBeing to take the RealAge Health Assessment.

 

Complete a medical or mental health care appointment between December 1, 2025, and November 30, 2026. You will need to manually confirm completion in your online CareFirst WellBeing account under Rewards.

Up your financial game by taking a Financial Finesse class between January 1, 2026, and November 30, 2026. Visit Financial Finesse to take a class.

You must log in to CareFirst WellBeing to manually confirm your participation in at least one class to earn your reward.

EARN HEALTHY REWARDS

When you participate by November 30, 2026, you can earn:

UP TO $400

for you

+ UP TO $400

if your covered spouse/domestic partner completes the activities too

= EARN UP TO $800

in Healthy Rewards that are divided among your monthly paychecks next year, reducing your 2027 Nestlé medical plan costs

* You must be an active U.S. NesCARE benefits-eligible employee (Hourly regularly scheduled to work 30+ hours or Salaried working 20+ hours, as defined in the NesCARE SPD) with a first day of work on or before October 19, 2026. You and your spouse/domestic partner must be enrolled in a Nestlé medical plan in 2027, and you and your spouse/domestic partner must have registered for your CareFirst WellBeing account by November 30, 2026. Spouses/domestic partners who are benefits-eligible must be on record with the Nestlé HR Service Center by October 19, 2026. Healthy Rewards are divided per paycheck throughout 2027. If your employment ends during 2027, your Healthy Rewards credit will cease. Participants with a CareFirst WellBeing account with steps processed by CareFirst WellBeing by October 31, 2026, should start receiving Healthy Rewards in January 2027. Participants with a CareFirst WellBeing account with steps processed by CareFirst WellBeing no later than November 30, 2026, should start receiving Healthy Rewards in February 2027. The Healthy Rewards deadline is November 30, 2026, at 11:59 p.m. Eastern Time.
